RegulationsTransition Services means a coordinated set of activities for a student with a disability, designed within a results-oriented process, that is focused on improving the academic and functional achievement of the student with a disability to facilitate the student's movement from school to post-school activities, including, but not limited to, post-secondary education, vocational education, integrated employment (including supported employment), continuing and adult education, adult services, independent living, or community participation. |

From Part 200, Section 200.4: Procedures for referral, evaluation, individualized education program (IEP) development, placement and review 200.4(b)(6)(vii) the student is assessed in all areas related to the suspected disability, including, where appropriate, health, vision, hearing, social and emotional status, general intelligence, academic performance, vocational skills, communicative status and motor abilities; 200.4(b)(6)(viii) students age 12 and those referred to special education for the first time who are age 12 and over, shall receive an assessment that includes a review of school records and teacher assessments, and parent and student interviews to determine vocational skills, aptitudes and interests; Helpful Websites: Conducting a Level I Career Assessment a guide to implementing the Level 1 vocational assessment, including directions and forms. Career Zone provides tools to use in career exploration, resumé writing and job searching. Career and Technical Skills Achievement Profile is a model Profile for students with disabilities who are pursuing an Individualized Education Program (IEP) Diploma. The Profile documents a student's attainment of career and technical knowledge and skills; work-related skills and certification or licenses and was jointly developed by the Offices of Curriculum and Instructional Support and Vocational and Educational Services for Individuals with Disabilities (VESID).
